UCAR Policy 2-4 "Investigator Financial Disclosure Policy" and its related procedures have been modified. The updated Policy incorporates reporting obligations recently instituted by the Department of Health and Human Services. The procedures, review guidelines, and the disclosure form related to Policy 2-4 have been modified to provide more clarity regarding what interests do and do not amount to Significant Financial Interests and/or Conflicts of Interest under federal law.  The revised Disclosure of Significant Financial Interests Form is now available in the forms library. Please discontinue use of the old form.
