No registration required.
This event is open to UCAR staff/retirees, their spouses/partners and dependent children 18-26 years of age. A sampling of Health Fair events include: Blood Screening Results and Private Consultations; Blood Pressure Screening, Full Blood Chemistry, PSA Prostate Cancer Screening; Optional Blood Screening Screenings; Pulmonary Screening; Bone Density Screening; Body Mass Analysis; Skin Cancer Screening; Vision/Hearing Screening; Ergonomics; Child and Elder Care Resources; Nutrition Resources; Benefits Information; Health Club Resources; Chair Massage, Accupuncture and Chiropractic Resources; Cigna & Kaiser Reps; Health Information Booths, Refreshments, Giveaways; Prizes; and much, much more!
Can't attend?? ALL unclaimed blood work results will be postal-mailed to participants. To designate a representative to claim your results on your behalf at the health fair, please complete the release form at: http://www.fin.ucar.edu/wellness/blood_screen_release.pdf. Your representative must bring this signed authorization form to the health fair -- no results will be released to without a signed consent form.
Please note: A blood-draw make-up session is scheduled for March 15, 2012, 8:00-10:00 am, at RAF/Jeffco. A 12-hour fast and pre-registration is required. Schedule an appointment at: Health Promotions registration site: http://www.hpmi.us/ucar.php. Bloodwork results from the 3/15/12 session will be postal-mailed to all participants.
