This is the final onsite seasonal flu vaccination clinic for the 2010 season. This event is open to UCAR staff, retirees, their spouses/partners and dependent children 18-25 yrs of age.
Can't attend but still need a flu shot? UCAR employees/retirees and eligible dependents can get the seasonal flu shot, or seasonal FluMist upon request and $19/pp co-pay to be paid by participants, at any of the following Passport Health locations through December 31, 2010 (or while supplies last). UCAR employees can use their UCAR ID/Access Cards (no voucher required) at the Boulder-Passport Health location. Vouchers are required for UCAR retirees, eligible dependents and at all out-of-Colorado Passport Health locations.
